Cardiology or Pulmonology Nurse Practitioner – Post-Acute Care (South Houston, TX)

About Us
CardioRenal Vision is an established, rapidly growing specialty practice providing cardiology, nephrology, and pulmonology care in the post-acute setting. We're pioneering specialty medicine in skilled nursing and long-term care, and we're looking for a Nurse Practitioner who wants to help shape what that care looks like.

The Role
We're hiring a full-time Cardiology or Pulmonology Nurse Practitioner to care for sub-acute and long-term care patients across facilities in the south Houston area. You'll work in collaboration with a supervising physician while managing your own patient panel, backed by comprehensive training and full clinical support.

What You'll Do

• Provide direct patient care, including comprehensive assessments, treatment planning, and medication management
• Document accurately in our EHR to ensure continuity of care across facilities
• Collaborate with facility staff, physicians, and the broader care team to deliver coordinated, comprehensive care
• Stay current on advances and best practices in cardiology or pulmonology
• Communicate with empathy and professionalism with patients, families, and colleagues
